PASSPORT APPLICATION

  • Personal appearance for all
  • Passport processing time is 20 working days
  • Passports unclaimed after six months from date of issuance will cancelled and disposed of per Department Order no. 37-03

REQUIREMENTS FOR RENEWAL OF PASSPORT

  1.  Duly accomplished passport application form
  2. Latest Passport and photocopy of first and last page with latest visa. If amended, include page with amendment.
  3. Photocopy of Macau ID/Blue Card (back to back). 
  4. Photocopy of valid passport of parents, if minor. 
  5. Three (3) photos (royal blue background)
  6. Personal appearance of the applicant is required.




IN CASE OF LOST PASSPORT

After submission of documentary requirements, there is a prescribed clearing period of 15 days prior to issuance of replacement. All passports not issued in PCG Macau shall be replaced only upon receipt of verification from DFA-Manila.

  1. Affidavit of Loss (2 copies)

2.       Police Report

3.       copy of previous passport/Philippine issued ID

4.       Copy of Ticket/Certification of confirmed flight to the Philippines.

5.       Four (4) photos

·              Contract Workers/Residents

1.       Affidavit of Loss (2 copies)

2.       Police Report

3.       Copy of lost passport (if not available, authenticated birth certificate)

4.       Authenticated marriage contract/Report of marriage if lost passport is under married name.

5.       Copy of valid employment contract and Macau ID

6.       Photo requirements and personal appearance shall apply

 

REQUIREMENTS FOR NEW PASSPORT (FOR CHILDREN BORN IN MACAU TO FILIPINO PARENTS)

  1. Duly accomplished application form (available at the Philippine Consulate)
  2. Copy of Report of Birth (filed with the Philippine Consulate General)
  3. Copy of birth certificate issued by the Macau Civil Registry Unit (translated into English)
  4. Copy of parent's passports and/or Macau ID
  5. Affidavit of Late Registration of Birth for births reported after twelve (12) months from occurrence (effective Jan 2008).
  6. Three (3) photos (royal blue background)

*Please present all the original copy of the documents mentioned

 

FOR AMENDMENT OF PASSPORT

 

Female applicants who wish to adopt their husband's surname

For those married in the Philippines

1.       Duly accomplished application form

2.       DFA authenticated marriage contract

3.       DFA authenticated birth certificate

For those married in Macau (please file Report of Marriage first)

1.       Duly accomplished application form

2.       Copy of Report of Marriage

3.       Copy of marriage contract (authenticated by the Philippine Mission nearest to the place where marriage was solemnized)

4.       Copy of DFA authenticated birth certificate of wife

5.       Copy of passports of husband and wife

6.       Personal appearance

 

Female applicants who wish to revert to their maiden name due to death of their spouse or annulment of marriage

·              For Widows

1.         DFA authenticated death certificate of husband

2.         DFA authenticated copy of previous marriage

3.         In case of presumptive death of husband, Court Order of Presumptive Death duly authenticated by DFA

·              For Annulled Marriages


1.       DFA authenticated marriage contract with annotation on annulment

2.       Certified true copy of Court Order and Certificate of Finality duly authenticated by DFA.

3.       DFA authenticated birth certificate of wife

·              For Divorcees


1.       Court Decision on Divorce Petition (petitioner should be the foreign spouse. In case petitioner is the Filipino spouse, item no. VII letter b shall be required)

2.       DFA authenticated birth certificate of wife

·              Divorce among Filipino Muslims

1.       Divorce Decree from Shariah Court

2.      Joint Affidavit executed by two Imams who witnessed the divorce declaration
